Madu sebagai Agen Oral Care pada Pasien Terventilasi Mekanik di ICU: Narrative Review
Abstract
Pasien kritis dengan penggunaan ventilasi mekanik di ICU rentan mengalami gangguan pada kesehatan mulut sehingga beresiko mengembangkan kejadian Ventilator Associated Pneumonia (VAP). Oral care menggunakan madu berpotensi efektif dalam menghambat kolonisasi mikroba patogen di mulut, namun penelitian yang membahas oral care madu di area critical care masih terbatas. Artikel ini bertujuan untuk menilai efektivitas oral care madu pada pasien terventilasi mekanik di ICU. Kami melakukan narrative review terhadap artikel yang dipublikasikan pada tahun 2011-2021 dengan menggunakan empat (4) database yaitu Medline (Pubmed), CINAHL (Ebsco), Science Direct dan Google Scholar untuk artikel yang berfokus pada penggunaan madu sebagai agen oral care pada pasien terventilasi mekanik. Tiga independent reviewers menganalisa berdasarkan kriteria inklusi dan eksklusi yang telah ditetapkan. Melalui pencarian data dasar diperoleh 2.514 artikel. Berdasarkan PRISMA flowchart dan setelah tinjauan manual hanya 7 penelitian yang sesuai dan diidentifikasi. Hasil telaah menunjukkan bahwa madu yang digunakan 2 kali sehari sebagai agen oral care dengan cara berkumur maupun teknik oles terbukti efektif untuk menghambat kolonisasi bakteri penyebab infeksi mulut; mengatasi berbagai masalah mulut pasien terventilasi mekanik seperti xerostomia, mukositis dan gangguan keasaman pH, serta berpotensi mencegah kejadian VAP.
